Canon IPF5100 print issue double printing

Has anybody experienced a similar issue with their prints.... I am trying to diagnose what the issue could be. Someone has said it looks more like a mechanical issue than a printhead issue. Be great if someone could say its likely to be x or y if that makes sense.

Left Printhead brand new
Service mode nozzle 1 check looks excellent.

Run all the adjustments.

I have attached some pictures of the problem....

Black motorcycle - underneath the bike

Hi Have you tried : paper advance calibration?
