Xerox Color 560 printing at wrong size/scale. Help plz.

I'm working on printing a drink menu for a local restaurant. These will be 8.5"x5.5" menus. I setup everything through Acrobat, and send it to my Fiery Command Workstation. Everything looks correct, but when I print, the image is offset and the wrong size, and the whole image not even on the paper. I'll post attachments to show my progression. I triple checked all settings and can't figure out what's wrong. I need help.

So after pulling out a lot of my hair and having a few choice words with Mr. Xerox Paininmyass, I told it to scale to fit media, it just made the whole thing smaller, though it did fit on the paper. Still unacceptable.

Perhaps you should not use the "fit" option.
Use scale option in CWS instead of PDF maker menu.
