Problem with mentor tacho failure

We have a random problem with KBA 162

Mentor gives error 86 tacho failure at start-up

We changed tacho and the cable and still have the problem randomly

Any thoughts ?

Its better to share little more details, at least the machine manufacturing year and mentor, picture, error picture at machine computer.
