PlateRite 8600MII-S Plate not centered issue

One of my operators was making plate when suddenly the plate that came out was not centered. He did not change the setting or make any changes to the platerite machine. Is this a mechanical issue or technical issue with the calibration because no changes was made to the setting of the machine. Pictures below for context.
