Could be a PIP underlayment issue.

What about the suction cups for the tail of the PIP? Oil teardrops can form if the tail of the pip is being pressed too hard down on the suction cups.

Is the defect exactly the same shape and location after PIP, PIP underlayer and blanket replacements?

Ameteks functionality means if they are working or working enough. But for me it still looks like underlayer. Maybe you did something wrong?

Edit: You can check also separated gray20, check if it's on one color or on one half of PIP. You need to look where it comes from.

If this still shows up after pip, blanket and underlay replacement the cylinder itself could be damaged.

When you are Printing Gray 20 please change ink order to See if that spot Will change colour. If Yes, propably you have weak spot somewhere, it happens sometimes and check if all drums are synchronized.

If it would be ITM drum issue would be visible on every separation, not every two. If its Old Press can be weak spot on PIP drum. First do the baseline, take out PIP and underlayer and check drum by fingers.

In the photo where you see only magenta at the bottom. You shoul have ymck as is grey. But you only have 1 color and not 2 as you say. Also this problems on the trailing edge normaly are because of itm Drum.
