If the ITB didn't show some major, obvious defect, i don't believe the belt is the problem. And i've never seen a liner void of that nature from a transfer belt. It's appearing throughout all colors. Check the fuser and see if you see any serious scratches on it. I was leaning toward the transfer belt myself, but since it seems to picking up toner and re-depositing it, fuser may be right.

Could be the subsequent toner hopper or auger that's failing. It's not moving toner to the delivery area, so it thinks it's out of toner.

From what I understand this error goes back quite some time. There should already be an existing patch to fix what Xerox calls a "programming error". I'm no longer a Xerox user, but I would search the error code on the xerox support page.
