I have cleaned the head several times and I still get this. Has this happened to anyone?
    Thank you,

    Hi Lisa,
the next step you can try is to execute "Power cleaning" from the Epson´s menu. Usually it should be solved then. After three unsuccessful power cleans better call your local Epson dealer for service.

Another option to try (if you haven't already spoke to Epson) is make a small box in the software you use and look up the color code for Cyan for your printer online. Put that code in the software color picker and print it a few times. This should have the printer attempt to use that specific color to try pushing the ink through the clogged nozzles alone. If that doesn't work, it's most likely a major clog problem that Epson would have to fix or hopefully not need to replace the head itself. Unfortunately, out of warranty will be pretty costly for this process.

From your photo I can suggest that you have physical problem with head maybe head have scratch if you clean many time and don't have any change in good or bad side I mean that scratch. If you don't know how manually move carriage and check head you must call service engineer. If you know how make this check head first. Good luck.
