we have this problem too, i'm fairly sure its due to heat stressing the paper through the printer first then the laminator.

It's all about heat, missing moisture in the paper, film quality and tension on the film. For one-side laminating we use the gbc 620 laminator, it has a "de-curler", which is no more than a contra-tension-bar over which the sheet is run before separating. The machine is quite expensive, tricky to handle and the separator needs a lot of experience to be set up for different materials. But it works, especially on book covers.

Film should be a "digital-lay-flat" type like the one gbc offers.

Nylon lay flat film is a great option for one side and two side laminating when flat matters. Polyester isa good film for two sided and most laminaing pouches ae made from this base as well.

You can use cold roll laminator. Cold roll laminators are simple laminating machines. Lamination process in these laminators is easy and faster. Laminating supplies used in cold roll laminator have one side pressure sensitive adhesive. Several prints are sensitive to heat and we cannot put them in high temperature contact. Such laminated materials undergo cold laminating.
Wax printing is an example where cold lamination is done. It use pressure sensitive laminating adhesives for sticking of film with substrate. Process is very simple and anybody can do that without taking any training. Cold roll laminator has one roller, which works as a pressure roll. When film and sheet passes through this roll, adhesive generate a bond with sheet due to the effect of pressure.
Cold roll laminator doesn't use heat to laminate sheets. Only pressure is the critical parameter which plays a role for laminating bond strength.

I have had the same issue and the above is what we did with the de-curl bar, but we also let the cover sit for 24 hours under pressure and they were perfectly flat.
