I had a bad one once where I put it in right out of the box. One thousand clicks later I hear THE BAD NOISE and find something like that picture in my fuser compartment as well as all the rubber torn and broken to bits all down the 5a to 9 areas.

Usually poor manufacturing. If the rubber isn't cured correctly there are air bubbles left in the mix. When you first heat up the roll it finishes off the curing process, the air expands and the rubber de-laminates from the metal core.

My iGen gets hungry for rollers every day, its common, just replace it, if it happens more than once in a day, you may have a temperature problem, then id call service and make sure the thermasters are clean.
